Introducción. La hipertensión enmascarada (HE) en niños se define por registros de presión arterial normal en la consulta y elevados fuera de ella. El monitoreo ambulatorio de presión arterial de 24 horas (MAPA) permite su diagnóstico. Existe poca información sobre su prevalencia en nuestra población. Objetivo. Estimar la prevalencia de HE en niños con factores de riesgo para el desarrollo de hipertensión arterial. Material y método. Estudio prospectivo, observacional, corte transversal. Se incluyeron en forma consecutiva pacientes asistidos en el Hospital General de Niños P. de Elizalde entre 1/7/2015 y 1/12/2016, de 5 a 11 años, con presión arterial normal y, al menos, un factor de riesgo para hipertensión arterial. Se realizó MAPA durante 24 horas (SpaceLabs 90207/90217). Muestra estimada: 110 pacientes. Se contó con las aprobaciones pertinentes. Resultados. Se incluyeron 110 pacientes, edad de 8,7 ± 1,8 años, 60 mujeres. Tiempo de MAPA: 23,18 ± 1,8 horas. Tenían antecedentes neonatales 23 pacientes; todos tenían, al menos, un antecedente personal (los más frecuentes, ingesta aumentada de sal y obesidad); 101 tenían, al menos, un antecedente familiar. El MAPA permitió identificar a 10 pacientes con HE (9,1%; IC 95%: 5,1-15,9); 7 con hipertensión nocturna aislada (6,4%; IC 95%: 3,1-23,5) y 28, prehipertensión (25,4%; IC 95%: 18,2-34,3). Los 10 pacientes con HE fueron 7 varones, 9 obesos y con, al menos, un antecedente familiar. Conclusión. La prevalencia de HE en niños con factores de riesgo de desarrollar hipertensión arterial fue cercana al 10%.

Among the 10 patients with MH, 7 were boys, 9 were obese and had at least one factor corresponding to family history. Conclusion. The prevalence of MH in children with risk factors for arterial hypertension was close to 10%.

INTRODUCTION: Despite many cases of pneumonia in children are due to viruses, initial management includes antibiotics. To avoid unnecessary antibiotic use, a clinical rule to predict etiology in children hospitalized for pneumonia was developed (BPS: Bacterial Pneumonia Score), but it performance in ambulatory settings has not been yet tested. We aimed to evaluate the safety of using BPS in children with pneumonia in an ambulatory setting. METHODS: This cohort study included children aged 3-60 months, assisted for pneumonias as outpatients. BPS (range: -3 to 15 points) was calculated in all of them, and only those with ≥ 4 points received antibiotics. Clinical outcome was evaluated 1, 2, 3, 5, 7, and 10 days after the first visit. RESULTS: 108 patients were included, aged 25.3 ± 16.5 months. From them, 48 (44.4%) showed BPS ≥ 4, and received antibiotics. After 10 days, there were no differences in clinical outcome between groups (with and without antibiotics) (p = 0.5; RR: 0.39; IC 95%: 0.01-5.74). Only 3 patients showed unsatisfactory clinical outcome; 1 of them had not received antibiotics. CONCLUSIONS: Using BPS was safe for initial management of children with pneumonia in an outpatient setting.

Introducción. Una parte sustancial de las neumonías en la infancia son de etiología viral, pero su manejo inicial habitual incluye antibióticos.Una regla para predecir etiología de la neumonía en niños hospitalizados (BPS: Bacterial Pneumonia Score) podría limitar este empleo innecesariode antibióticos, pero su utilidad no ha sido evaluada en pacientes ambulatorios. El objetivo del presente trabajo fue evaluar la seguridad dela aplicación de una regla de predicción para el manejo inicial de pacientes con neumonía asistidos en forma ambulatoria.Material y métodos. Estudio de cohorte que incluyó pacientes de 3-60 meses de edad asistidos ambulatoriamente por neumonía. Se calculó elBPS (intervalo: de -3 a 15 puntos) en todos los pacientes y sólo en aquellos con BPS ≥ 4 puntos se indicó antibiótico. Se evaluó la evolución clínica1, 2, 3, 5, 7 y 10 días después de la visita inicial.Resultados. Se incluyeron 108 pacientes, de 25,3 más sobre menos 16,5 meses de edad. En 48 pacientes (44,4 por ciento)el BPS fue ≥ 4 puntos, por lo que se indicó antibióticos.No se encontraron diferencias en laevolución clínica entre ambos grupos (con antibióticos y sin ellos) al décimo día (p= 0,5; RR: 0,39; IC 95 por ciento: 0,01-5,74). Sólo tres pacientes presentaron mala evolución clínica; 1 pertenecía al grupo que no recibía antibióticos.Conclusión. El BPS se mostró seguro para la toma de decisiones en la consulta inicial en niños conneumonía de manejo ambulatorio.

RESUMO

Fueron tratados treinta niños de uno u otro sexo, con edades entre 1 y 11 años, con infecciones de garganta por Estreptococo beta hemolítico del grupo A, con etilsuccinato de eritromicina por vía bucal cada ocho horas con dosis promedio de 50 mg/kg día. Veinte pacientes (66.6%) tuvieron resultados excelentes (mejoría dentro de las 48 horas de tratamiento). Nueve niños (30%) tuvieron buenos resultados (mejoría dentro de las 96 horas de tratamiento). Un sólo niño (3.3%) tuvo resultado regular (curación entre el cuarto y séptimo día de tratamiento). Con esto se concluye que hubo resultados favorables en todos los pacientes. La sensibilidad del estreptococo beta hemolítico a eritromicina fue confirmada en el 100% de los cultivos. No hubo casos de resistencia. La tolerancia fue óptima porque no se registraron efectos indeseables que ameritaran suspender el tratamiento
